Sometimes we never find out what caused a problem. I would just put it all back together, making sure that everything fits correctly, and run it. You've seen signs of unusual wear but there's no confirmed connection to any running problems.

I just bought a Standard Motor Parts cap and rotor. I figure if anything would fix the issue since there is no shaft play it would be buying both parts made by the same company.
