I started a new thread for the engine diagnosis. I don't plan to update this one any more.

EDIT: For the sake of closure, the engine runs now.  Turns out I didn't properly seat the #5 fuel injector seal when I replaced it.  This caused a huge vacuum leak which was causing most of the issues.  See above linked thread for details.
